– once registered, and email is confirmed, you can request in person visit or telemedicine consultation at your convenience.
Important!
All NEW patients to Be Well Endocrinology need to complete clinic forms and questionnaires assigned to new patients. Dr. Andzel will review all information provided BEFORE the consultation what helps with formulating a management plan. If you have medical records to share please submit files on the portal or email info.
